Payment is due at the time of your appointment. I accept cash or checks. If you are making a co-payment, please try to use cash.
First appointment requires paper work to be completed before the session begins. You can either arrive about 15 minutes early in order to fill in these forms or you can download them and bring them in at the time of your first appointment completed.
If you are using your Blue Shield Blue Cross insurance, please bring the card so that I may make a copy of it.
I am a participating provider for CareFirst BlueCross BlueShield plans (not Blue Choice).
For other forms of insurance, I am an out-of-network provider. I will provide a receipt of payment that you can submit to your insurance company for reimbursement. Please check with your insurance company prior to our first session to learn more about your out-of-network benefits.
An Important Conversation to Have with Your Insurance Company, Even If you’re Pretty Sure You Understand Your Policy, In Order to Avoid Unpleasant Surprises Later:
- “I’m planning to schedule an appointment for outpatient psychotherapy in an office-based setting. Is Jamesen Goodman, LCPC a participating provider for my insurance plan?”
- It’s important to mention that you are seeking mental health/behavioral health treatment because it may be handled differently than other health care services. In some cases, mental health coverage is carved out (subcontracted) to another company. You may need to speak to that second company directly to learn about your coverage for mental health treatment.
- If I am an in-network provider for your insurance plan: “What will my copay be when I see Jamesen Goodman? Is there a deductible that I will have to meet before the insurance company will begin to pay for these services? If so, how much of my deductible is remaining for this calendar year?”
- If I am not an in-network provider for your insurance plan: “Will I be able to get any out-of-network reimbursement for my sessions with Jamesen Goodman if I can provide a detailed statement? If so, how is the reimbursement calculated? Is there a deductible that I will have to meet before the insurance company will begin to pay for theses services?”
I will not share your personal information without your permission. However, state law and professional ethics require therapists report in the following situations: suspected past or present abuse or neglect of children, adults with special needs, and elders to the authorities, including Child Protective Services and law enforcement, based on information provided by the client or collateral sources OR if I suspect the client is seriously in danger of harming him/herself or has threatened to harm another person.
Please provide me with 24-hour notice to cancel an appointment; a fee will be charged if notice is not given.
